First confirm the client supports the Reality protocol (Surge and Quantumult X do not). Then check that you've selected a node on the carrier route that matches your own ISP — using an Unicom-optimized node on a Telecom connection, for example, will underperform.
Yes — the carrier-aware routing simply reflects which network you're currently on. If your ISP changes, reconnecting will resolve to the best-performing path for the new network automatically.
